This Assessment is a tool provided to your financial advisor or potential financial advisor (“Advisor”) by C & J Innovations, who will both receive the data that you provide. C & J Innovations values your privacy and has established this Online Privacy Policy (“Policy”) that describes how we use, disclose, and protect the information that we collect online from visitors to our Assessment. This Policy is separate and apart from the Privacy Policy of this website and/or the Privacy Policy provided by your Advisor.

Collection of Personal Information

The scope of information that we collect from you may vary depending upon your interaction with the Assessment. Your Advisor and C & J Innovations will have access to this information.

This Terms of Use Agreement governs your access and use of the C & J Innovations’ Assessment and the financial planning information and tools provided thereon. These Terms of Use are separate and apart from the Terms of Use for this website and/or Terms of Use provided by your Advisor. Your Advisor and C & J Innovations will have access to this data.

Ownership and Purpose of the Assessment

—C & J Innovations owns and operates this Assessment and controls the financial planning information and tools provided thereon, which are collectively referred to herein as the “Services.” The purpose of the Services is to provide financial planning information, tools, and assessments to visitors or users of the Assessment. The Assessment and the tools provided thereon may also provide the visitor or users with recommendations of financial products or services.

—BY USING THE SERVICES, YOU AGREE TO THESE TERMS OF USE. —The Services are intended to be used by visitors or users that are interested in accessing the financial planning information, tools, and assessments and corresponding recommendations provided through the Assessment. To begin use of the Services, you may be asked to provide contact information about yourself. Your Advisor and C & J Innovations will have access to the data you provided. The Online Privacy Policy addresses how the information that we collect from you will be treated.

—The information that you provide through your interaction with the Assessment must be your personal information. You may not enter personal information of another individual into the Assessment.

Violation of the Terms of Use

—You agree that C & J Innovations may, in its sole discretion and without prior notice, terminate your access to the Services and/or the Assessment for violation of these Terms of Use.

Modification or Termination of the Assessment
